CORONAVIRUS (COVID-19) ADVICE
PLEASE DO NOT ATTEND THE GP PRACTICE IF:
- you think you might have coronavirus
- you’ve been in close contact with someone with coronavirus
- you have had a raised temperature, loss of sense of smell/taste, cough, cold, sore throat, exhaustion.
NHS 111 has an online coronavirus service https://111.nhs.uk/covid-19 that can tell you if you need medical help and advise you what to do.
The GPs at the Practice have no additional infomation than can be found at this website and beyond this are unable to help with requests for sick notes from the employers of the extremely clinically vulnerable group of patients.
For parents concerned about children attending school, we would suggest a discussion with the school would be the best course of action.
For the latest Covid-19 advice please visit www.nhs.uk.
The Islington GP Practices are working together to vaccinate our population. The Rise Group Practice will be informed of an upcoming clinic and the group of patients eligible for the clinic.
We will then call the eligible patients to book them in. We have no further information at this time and would ask that you do not call reception for an update.
To ensure we can contact you to offer you the vaccine, please email email@example.com with your contact number(s) and email address, if you think our records may need to be updated. Please use the subject line 'Contact Number Update' so we can easily identify these emails.
We are now able to offer access for our Patients to e-Consult. You can request a prescription, fit note, or appointment, by filling in a questionnaire at https://therisegrouppractice.webgp.com and we will get back to you within 3 working days.
The website contains a range of health information and resources, along with practical information on visiting the surgery. We hope you find the website useful and informative

In the car park, attached to the surgery, a reserved car parking space for the disabled is marked near the front door. Wheelchair access to the building is via the front entrance. A disabled patient’s WC is provided. If access proves difficult to any of our disabled patients we would be happy to consider any suggestions for improvement. Disabled patients needing the car park are asked to telephone reception with their time of arrival and access will be arranged. The health centre car park is not available to patients. There is access for wheelchairs from on-street parking.
